Connect to Care
Veterans, service members, and their families experiencing difficulties in daily life or in distress can reach out to Check-In and trust that an advocate will find a licensed professional who can help.
If you answer yes to any of the listed questions, please consider reaching out.
- Do you find yourself having trouble focusing, concentrating, or remembering things?
- Are you struggling with sleep, headaches, loss of energy or appetite, or substance abuse?
- Have you experienced a disruptive life event such as a break-up or divorce, the death of a loved one, or the loss of a job?
- Do you feel angry, sad, frustrated, overwhelmed, or alone?
- Do you just want to improve your mental well-being and live a healthier life?
Here’s How To Get Started:
Step 1: Reach Out
The care journey begins by sign-up or referral. Once connected with Check-In, a trained “Care Coordinator” will be assigned to you or your loved one within 72 hours. The Care Coordinator will listen, answer questions, and begin the care process.
Step 2: Get Connected
After administering an initial intake screening, the Care Coordinator will develop a personalized care plan, matching you or your loved one’s unique needs to the programs and organizations best able to provide help.
Step 3: Receive Care
The Care Coordinator will then assist in scheduling care, with treatment options ranging from outpatient and telehealth programs to intensive, in-person care programs. The Care Coordinator will continue to serve as an advisor and guide throughout the care journey.
Am I able to receive care if I do not have an honorable discharge?
Yes, you and your loved ones are able to receive mental and brain health services regardless of your discharge and length of service. The Veteran Wellness Alliance will work with you to find a provider within our clinical partners that are able to serve your needs.
